OSHC Co-curricular and Events Notification Form NOTE: THIS IS NOT A BOOKING FORM- Bookings are added via the Kidsoft iParent Portal (after completing the enrolment). Complete this form to advise OSHC of the details of your daughter’s afternoon activities on the days she will attend OSHC so we know where she is at all times. This form provides authorisation/permissions as required under the Education and Care Services National Regulations (NSW) enabling OSHC staff to sign your child out of OSHC to attend the activity. This also advises should you collect your daughter straight from an activity and not be returning to OSHC. Contact OSHC Administration should you require any assistance. Information will only be recorded and retained for Routine/Regular Casual bookings for Term dates advised. I agree to resubmit this form if my child's activity times change. I understand my child will be unable to attend activities if I have not shared updated information with the OSHC Centre. (Years K- 2 only) I accept that my child will be attending and travelling to and from activities during an OSHC session and will be under the supervision of school walkers organised by Pymble Sport. I have also notified Prep Administration for the purpose of organising travel arrangements for my child. OR (Years 3-6 only) I accept that my child will be attending and travelling to and from activities during an OSHC session and will not be under the supervision of the Pymble LC OSHC staff. When my child is not returning to the OSHC Centre after the activity, I have notified OSHC administration and I authorise an OSHC staff member to sign my child out from the OSHC Centre. I give permission for my child to attend regular excursions within the grounds of Pymble Ladies' College when accompanied by OSHC Educators. I give permission for my child to participate in regular rehearsals for Emergency Lockdown and Evacuation procedures.
